School of Nursing ID Badges for Clinical Faculty
If you hold a clinical faculty appointment -- full or part-time -- you are eligible for a School of Nursing (SON) ID badge. This badge can be used in place of your SFSU faculty ID badge and will serve to identify you on all clinical sites as a member of the SFSU School of Nursing faculty.
If you would like a clinical ID badge please follow the guidelines listed below. Please note that requests should be submitted at least 3 business days prior to the date the ID badge is needed.
- Indicate the date by which you would like to receive the badge.
- Send a 1”x1” photo by postal mail or email to the appropriate address noted below.
- Provide your name and credentials as you would like them to appear on the badge.
- Provide your mailing address if you would like the ID badge sent to you.
- Provide your phone number if you would like to receive a call when the ID badge is ready for pick-up.
